Social Services & Admissions

Best of CareSocial services are provided to assure that each resident can attain or maintain the highest practical physical, mental and psycho-social well-being.  The Social Services Director can help with admission procedures, potential plan of care, advance directives, and designating a health care proxy.  

Nutrition Services

Dining services are open daily from 7:00 am to 7:00 pm.  A variety of delicious meal options are available in the dining room during designated service hours.   All meals, between-meal snacks, nutritional supplements and special diets meet American Dietary Association standards.  Food is prepared and served under the guidance of our certified dietary personnel.  Our Registered Dietician sets guidelines for the dietary department and reviews the nutritional needs of the residents, making dietary suggestions as needed. 

Activities

We offer a lively activities program to stimulate aging minds, encourage friendships, promote a sense of belonging, and foster residents’ self-esteem, creativity, education, and independence.  Our activities services take into consideration the preferences of our residents, including interests, hobbies, lifestyle and culture.  Group outings supervised by our Activities Director include trips to the baseball park, fishing at local lakes, exploring our historic hometown, or watching a local parade or other events.  We encourage families to attend social events in our facility, as well as outings on the town.

Moving to a health care facility for a short stay or a long stay can be a stressful experience for the entire family.  We aim to make the admission process for a new resident as easy as possible. The Administrator and staff will assist new residents and their family to be fully informed of admission processes, contractual documents, facility policies, and orientation to our facility and the daily routine. 

The following documents are required prior to a resident’s admission to the facility:

  • Physician's Certification Form (stating that the resident is appropriate for placement)
  • Complete list of all current medications
  • Copy of Social Security, Medicare, and Insurance cards
  • Copy of Advance Directives, Living Will, Power of Attorney, etc.
  • Phone numbers and addresses of primary contact person(s)
